Medication

If your child requires medication in school you must fill out a School Medication Authorization form. This needs to be signed by a medical prescriber (MD or APRN) and by a parent/guardian. The medication must be brought in by an adult, in a new, unopened box. This includes Acetaminophen and Ibuprofen. I also need the original packaging/box. I cannot give the children cough drops in school and they are no longer allowed to bring them in due to the choking hazard and the high-sugar content.
