1. Permission to Attend Form - this is a sample form. In order for athletes to attend a overnight/travel trip, a form needs to be developed by the coaches or managers of the team with all relevant information, please include coaching staff names, contact  and time/location/accommodations/expectations etc.  

2. Volunteer Driver Application Form - If students are going to be travelling to the overnight/tournament in a vehicle with a volunteer driver then this form has to be filled out by that volunteer driver and submitted by the coach or manager to administration for approval. *NOTE an up to date vulnerable sector check is a requirement for any volunteer driver transporting students.

3. Permission to Travel with Volunteer Driver (Student Form) - This form will need to be developed by the coach or manager. It needs to be be used for students who will be travelling in a vehicle with a volunteer driver associated with that team for a particular event. Coaches/managers will need to collect these forms and hand them into administration for approval prior to any student travelling.
