Hospital Acquired Infections/Antibiotic Resistant organisms (HAI’s/AR) require special reporting to the Kentucky Department of Public Health via an EPID250. These forms can be faxed to KDPH at 502-398-2462.
Some HAI’s/AR’s are required to be reported within one business day. They are as follows:
Carbapenem-resistant, Acinetobacter, Enterobacteriales (Enterobacteriaceae), and two variants of Pseudomonas Vancomycin-intermediate Staphylococcus aureus (VISA) and Vancomycin-resistant Staphylococcus aureus (VRSA) and Candida auris*.
For some illnesses, no EPID 250 is required instead, Electronic Lab Reporting, sent within five days is permissible. These illnesses are as follows:
Clostridioides difficile (C. difficile)
Enterobacteriales (Enterobacteriaceae) species resistant to ceftazidime, ceftriaxone or cefotaxime (ESBL)
Methicillin-resistant Staphylococcus aureus (MRSA)
Vancomycin-resistant Enterococcus species (VRE)
