There are two ways to provide the survey to participants:
As a printed copy (view PDF, not for printing) which come lined with a sealable gum. Simply moisten, stick the survey together and post using the freepost address on the rear. See details of how to obtain the survey below. Large print and easy read surveys can be printed using the PDF links below
Online, using the links below. You can provide this postcard to participants on how to complete the survey online. Use this template when sending emails to participants asking them to complete the survey.
