What is a Section 504 plan?
A section 504 plan is a legal document that provides individualized supports for students to help them be successful navigating a disability in the classroom.
What conditions could qualify my child?
Any condition that limits one or more major life activities may qualify. Examples may include: depression, anxiety, ADHD, IBS, diabetes, epilepsy, etc.
How is a 504 plan different from an IEP?
A section 504 plan is list of accommodations to use in the classroom, but does NOT include specially designed instruction (time outside of the classroom in the Learning Resource Center).
What is the process for being evaluated for a 504 plan?
Once the school receives parental consent for educational screening, data is collected. After, a team meeting is held to see if the student qualifies. If so, a plan is written with the team.
What data is collected?
Each scenario is a little bit different. Data may include: medical records, classroom observation, teacher input, parent input, student interview, discipline referrals, attendance, etc.
How do you get the medical statement?
There are two options: 1.) You call the doctor to request a medical statement and provide it to us. 2.) The school communicates directly with the doctor's office after getting a "release of information" signature from you.
Who is on the educational team?
The team includes the school counselor, the student’s teacher, parents, principal, possibly the school nurse. If you wish, the child is welcome to be a part of the team too.
How often is the plan reviewed?
At minimum, the plan is reviewed annually. It is a living document, so the team can update it on an as needed basis. Every 3 years, there is a re-evaluation to see if your student still qualifies for a 504 plan.

What is a Prior Written Notice (PWN)?
A Prior Written Notice is a formal document explaining why the educational team did or did not find the student eligible for a 504.
