Athletic Registration
Parents/Guardians-To register your son/daughter for athletics at NCUHS please complete the steps below.
In order for athletes to be eligible to participate (including tryouts) you must fully complete the registration process. This includes the Policies and Procedures Form, the Athletic Participation Form and proof of a physical (Well Exam) within the past two years.
*Note- Pre-participation checklist- some local providers require this document in order for them to process a request for a Well Exam form.

Download a copy of the online forms submitted.
Complete all three forms.

Physical Upload Form (even if you have a valid physical on file)
Registrations completed over the summer will be approved in early August. To check the status of your registration, please log into your parent account.
