Please email the guidance administrative assistant at bgaumond@naschools.net to request your high school transcript.  

 Your request must include the following information:

• Your name (as it was when you graduated)

• Date of Birth

• Year of Graduation

• Where the transcript should be mailed (complete address) or if you are picking it up in person (have ID ready)

• Does it need to be Official or Unofficial

 PLEASE NOTE: we cannot accept transcript requests from anyone other than the actual graduate. 

Working papers for students working in Massachusetts are available in the Main Office, or may be downloaded and printed here.  

Once filled in by the EMPLOYER, PHYSICIAN (students aged 14-15 ONLY) and the PARENT/GUARDIAN, the student must return the forms IN PERSON to be processed.  

If the employee is not a student of NAHS, and cannot come into our Guidance office between the hours of 7AM -2:30PM, they should bring the forms to our central Administration Office located at 6 Morse Street, North Attleborough, MA 02760.
