The links below will provide you with the PDF forms required for every student that participates in the NJROTC Program.  These forms can be printed at your convenience.  Please use black ink and ensure that all questions are answered.  The Student Code of Conduct and Parent/Guardian Consent Form includes instructions located on page 4.  If any questions from the “Health Risk Screening Questionnaire” are marked “yes”, then a physician is required to sign the form.   All "Physical Forms" must be signed by a physician.  "Standard Release" forms must be filled out and signed by a parent/guardian.  Please contact us if you have any questions at 334-298-3626, ext 3040.  Email: rhabersham@pcboe.net; zperry@pcboe.net; nfenell@pcboe.net
