Immunizations:
despite the pandemic, getting your child immunized is crucial to their health and physical well being
please turn in all required immunizations to your school's health office staff asap
Physicals:
needed for enrollment into the first grade however, it can be turned in anywhere from 18 months before or up to 90 days after enrolling in first grade
note: If you turn in a physical before your child is 18 months from entering the first grade (such as with your TK enrollment packet, you will be asked to turn another one in that falls within the required window)
Chronic Health Issues
Please make sure to notify the school health office staff as soon as possible should your child have a health condition
Should your child need to take medications at school, please take the following form to your child's doctor
Self -Medication Permission Form - for students who's doctor approves their ability to carry and administer their own medication, such as an inhaler

NOTE: One medication permission form is required per medication
NOTE: Forms must be completely filled out to be accepted.
Vision:
Lynwood Unified School District works closely with Vision to Learn throughout the year to provide free glasses to students who need them
during the pandemic, Vision to Learn is not performing vision testing at this time

St. Francis Medical Center
Health Communities Initiative:
Brings health screenings, immunizations and health education directly to area schools, churches, business and comunity organizations, via it's HCI nurse, health educator, student nurses and the mobile unit. For families without access to primary care, HCI provides them with important preventative services and healthcare resources.
Volunteer Program
Delivers orientation, training and supervision to high school, college and internship program students. The program benefits the student volunteers by introducing them to future healthcare careers and providing job skills training as they bring administrative and customer service support to the hospital.
Vision to Learn
Provides free vision screening and vision testing to students throughout the school district.
Each school eligible for these free services every other year.
During this pandemic, current vision screening and testing services are suspended.
However, replacement of lost or broken glasses originally provided by Vision to Learn within one calendar year can be provided at no cost.
