Health Evaluation
All of our health information is gathered through Pratt Institute in Brooklyn. Please go to https://www.pratt.edu/about/offices/student-affairs/health-services/immunization-information/ to follow the instructions for submission.
This video will also guide you through this process.
Completed Health Evaluation Packets must be uploaded by June 15, 2024.
Please note:
If you take any medications for either physical or mental health, please make a plan to ensure that you have the medications you need throughout the semester.
We strongly advise that you arrive on campus with the medications you need
If you are an NY State resident, consider speaking with your prescriber about your refill size, or ask if they can send scripts to a pharmacy in Utica.
If you are not a NY resident, ask your prescriber if they can continue to provide you with medications across state lines, or if they can provide you with a multiple month refill to get you through the semester.
Please note that the waitlist for a psychiatrist in the Utica area is 2-3 months, so a disruption in psychiatric medications can be difficult to address without sufficient notice.
New York State requires that all students born on or after January 1, 1957 must prove immunity to Measles, Mumps, and Rubella. Students must also review information regarding the Meningococcal Meningitis vaccine and provide proof of vaccination or a signed refusal. Pratt requires that all students complete a tuberculosis screening and a physical examination.
The Health Evaluation Packet consists of three forms that must be completed by a medical healthcare provider.

Download and print out the packet and bring it with you to your medical provider’s office. Once you and your medical provider have completed all three parts, scan or photograph each part and upload them to the Pratt website.
