Health Evaluation

All of our health information is gathered through Pratt Institute in Brooklyn. Please go to https://www.pratt.edu/about/offices/student-affairs/health-services/immunization-information/ to follow the instructions for submission.

This video will also guide you through this process.

Completed Health Evaluation Packets must be uploaded by June 15, 2024

Please note:

If you take any medications for either physical or mental health, please make a plan to ensure that you have the medications you need throughout the semester.

New York State requires that all students born on or after January 1, 1957 must prove immunity to Measles, Mumps, and Rubella. Students must also review information regarding the Meningococcal Meningitis vaccine and provide proof of vaccination or a signed refusal. For more information and guidelines, see the immunization requirements for college entry in New York from the New York State Department of Health.

Pratt requires that all students complete a tuberculosis screening and a physical examination.

The Health Evaluation Packet consists of three forms that must be completed by a medical healthcare provider.

Download and print out the packet and bring it with you to your medical provider’s office. Once you and your medical provider have completed all three parts, scan or photograph each part and upload them to the Pratt website.
