All NHS forms are available below. You must print as many forms as you need for verification purposes. Remember, an endorsement form must be submitted for anything you list on your Student Activity form. The activity/service/leadership will not count if there is no such verification!
Qualifying students receive nomination letters via their school email. NHS is NOT responsible if students do not check their emails and miss their notification. It is a student's personal responsibility to check their emails on a daily basis.

A few things to consider when a student receives the final decision letter for NHS membership.
-Any student who receives an invitation to apply to NHS is exceptional! They would not have received the invitation to apply otherwise.
-Please realize that NHS is a club and not an academic requirement. It is not required, or necessary, to be a member of NHS to be accepted by any college or university. Additionally, acceptance into NHS is NOT required, or necessary, to win scholarships to any colleges/universities. NHS is an opportunity to serve one's community and to have additional opportunities to experience community leadership. It is NOT a gateway to college acceptance, or scholarship considerations. This publicly endorsed perception is INCORRECT and puts unnecessary pressure on our children.
-There are many considerations that go into play when the faculty council considers applications to NHS. These considerations are set by the National NHS organization, not by the Charles L. Daly Chapter or by the school district. These considerations are non-negotiable. If the Daly Chapter fails to uphold these considerations to the intended standards of National NHS, our charter to operate will be voided and the Daly Chapter will cease to exist at MWHS.
-If a junior is not accepted into NHS during their junior year, they will be re-invited again during their senior year, so long as they maintain the weighted 92 GPA required. It is encouraged that students meet with the faculty advisor to gain advice on how best to move forward with their senior year application.
-It is NOT a failure, nor is it a lack of endorsement of an applicant's quality, if they are not accepted into NHS the first time around. It is hard to earn NHS membership. If it was easy to earn NHS membership, acceptance would not be so prestigious. Many considerations must be meet. Everyone's co-curricular, service, leadership and character experiences differ. There are MANY, MANY students who appear to have the "right stuff" who are not accepted the first time around. Please refrain from equating an initially denied application to "failure" or "lack of quality" of a candidate. If they weren't superior from the start, they would never have been invited in the first place.
