Complete this Schedule Change Request Form if a course change is necessary during the first five days of the semester (9/1 by 2:30pm). (You must be logged into your MVSD account to access the form.)
Schedule change requests will only be reviewed for the following reasons.
Your schedule is incomplete (missing periods)
You have already passed a non-repeatable class
You do not meet the prerequisite for a requested class
You have two-courses during the same period or the same course in two periods
You are missing a graduation requirement
You are a Running Start student who needs to provide us with a copy of your Running Start courses in order to be removed from MVHS courses. (upload on this form)
Students who are missing a class period in their schedule should come to the Counseling Office during that open period for assistance.
Continue attending the classes listed in Skyward unless and until you are notified that your schedule has been changed.
Schedule Change Requests will not be reviewed until the first day of school.
Counselors will make every effort to review eligible requests within five school days.
MVHS does not make teacher-based schedule changes.
Many classes are full, so submitting a request does not guarantee that a change can be made.
Due to limited staffing, counselors will not be available for appointments regarding schedule change inquiries.
If your requested change requires an additional form (TA, Science Intern, Early Release/Late Arrival, etc.), submit the completed form to the Counseling Office.
Continue following your current Skyward schedule until you receive confirmation that the form has been processed and your schedule has been updated in Skyward. Please follow up to five business days for processing.
Teacher/Office Aide - 11th or 12th graders only
Early Release/Late Arrival - 12th graders only
AVID Intern - 11th or 12th graders only
Science Intern - 11th or 12th graders only
Zero Period Contract - Complete if planning to take Zero Period Course
