Insurance and Payment Info

Mustard Seed Therapy, PLLC accepts private pay / self-pay clients. We collect our service fees directly after the session. Please contact us for specific evaluation and treatment rates.

Currently, Mustard Seed Therapy PLLC is in-network with Blue Cross Blue Shield of Texas (PPO, HMO, Medicaid, STAR, etc.) and certified with Humana Military (Tricare East). Once we collect your insurance information, we will verify your coverage. After each session, we submit a claim to your insurance and collect your co-pay.

If we are not in-network with your insurance, we can provide you a super bill for reimbursement. You will be responsible for the service fee directly after the session. A super bill will be provided to you. You will then submit the super bill to your insurance. Your insurance will reimburse you depending on your out-of-network coverage.
