If you are a current student at NMHS and you need a copy of your transcript sent somewhere, please complete a Transcript Request Form and turn it in to Mrs. Miles.
Pick up a copy of the form from the table outside the counseling office, or you can download it here.
Please write clearly and fill in the correct information. Whatever you write down is where she will send it, so please be sure it is accurate.
If you are needing it sent to multiple locations, please fill out multiple forms. Only one request per form, please.
Once complete, turn the form in to the blue box outside of the counseling office.
If you graduated from NMHS, please visit parchment.com to request your transcript.
If you transferred to another high school from NMHS, your transcript will automatically be sent when your new school submits a records request. If you have questions about whether or not your records were sent, please contact Mrs. Miles at (706) 695-7760 ext. 5010.
If you are a DE student, you will need to submit both your high school and college transcripts when applying to schools. Follow the instructions above for requesting your high school transcript, and go to your DE college's website for instructions on how to request theirs.
