The Core Phonics Survey aligns well with our Scope and Sequence. Print pages 15 to 23 of this PDF to assess your student and identify the phonics patterns that are challenging for your student.
Pages 7 to 11 also include phonological awareness assessments.
DIBELS (Dynamic Indicators of Basic Literacy Skills) includes a variety of assessments designed for specific grade levels. According to their website, "They are designed to be short (one minute) fluency measures that can be used to regularly detect risk and monitor the development of early literacy and early reading skills in kindergarten through eighth grade."
With the input of your mentor teacher, complete the appropriate DIBELSs assessment to identify where your student needs support. Download the appropriate DIBELS documents.
Your assessment information should help you to plan your lessons to meet students' needs. Add your assessments to your tutoring log. Plan to reassess later to monitor progress.
Those in K-2 may wish to use Heggerty Phonemic Awareness Screener appropriate for your student (K, 1, or grade 2+). Save your assessment and repeat the assessment mid-year to monitor progress.
It it recommended that students who score within the "Beginning" range for any skill would receive additional support and intervention.

The Words Their Way Spelling Assessment shows which spelling patterns the student can write. This is an optional assessment with words sequenced in alignment with the scope and sequence and is likely most appropriate for grade 2+. The link provides detailed instructions for administering the assessment.
Ask your mentor teacher which assessments are best suited to your student. If the student is getting discouraged, it may be time to discontinue that assessment.
Remember to print out the assessment tools you plan to use before arriving at the school.
