This document needs to be on file for all students completing the assessment. A copy should be in the test environment and another copy filed in their cumulative folder.

Sign the Form
This Assurance of Test Security Form must be signed after training is complete by all adults in the testing environment. They stay on file with the building administrator.
1) Download and complete the fillable PDF
2) Save as, add your name (example: TA_assurance_form_JDoe.pdf)
3) Email to you administrator
