This document needs to be on file for all students completing the assessment that require a Student Accessibility Support Plan. A copy should be in the test environment and another copy filed in their cumulative folder.
This exemption request applies only to the Oregon Science, Oregon ELPA Screener, Oregon ELPA Summative, and ORExt Science Assessment. It does not apply to other state or district assessments or learning activities.
Under Oregon Administrative Rule 581-022-1910, a “school district may excuse students from a state required program or learning activity, where necessary, to accommodate students’ disabilities or religious beliefs.”
This Assurance of Test Security document must be signed after training is complete by all adults in the testing environment. They stay on file with the building administrator.

This test is required for all Test Coordinators and Test Administrators. Upon completion of this form the District Testing Coordinator will update the users TIDE account.
