All children enrolled in a public or private school in Maine must have the following immunizations:
Required for PreK entry:
4 DTaP (diphtheria, tetanus, and pertussis)
3 Polio
1 MMR (measles, mumps, rubella)
1 Varicella (chickenpox)
Required for Kindergarten entry:
5 DTaP (4 DTaP if 4th is given on or after 4th birthday)
4 Polio (3 Polio if the 3rd is given on or after the 4th birthday)
2 MMR (measles, mumps, rubella)
2 Varicella (chickenpox)
Required for 7th grade entry:
All Previous vaccinations
1 Tdap (diphtheria, tetanus, and pertussis)
1 MCV4 (Meningococcal Conjugate Vaccine)
Required for 12th grade entry:
All Previous vaccinations
2 MCV4; only one dose is required if the 1st dose is given on or after 16th birthday
Proof of each administration must be provided to your school nurse, and include vaccine type, date administered and provider name.

Maine State Law allows for a one-time 90-day period from the time of initial school registration, and allows for 21 days to transfer health records for a student enrolling in a new school. Following this time period, the superintendent is not permitted by state law to allow students to attend school who are not adequately immunized and an exclusion notice will be sent to you.
