Vision and Hearing Screenings

Vision and hearing screenings are conducted within the district yearly based on the grade your student is in. Screenings are done by certified vision and hearing screeners. Those screeners are Madeline Goldman, the health EOP at Middletown Prairie Elementary, and Tara Pawlak, the health EOP at Mahomet Seymour Junior High.

Vision Screenings are conducted annually for all:

-Middletown Prairie Students

-8th Grade Students

-Special Education Students

-Parent/Teacher Concerns and/or Requests

*If your student wears glasses/contacts, they will not have a vision screening done at school.

Hearing Screenings are conducted annually for all:

-Middletown Prairie Students

-3rd Grade Students

-Special Education Students

-Parent/Teacher Concerns and/or Requests

What if I have concerns regarding my student's vision and/or hearing?

Vision and hearing screenings are done once a year at each school for specific grades,

as mandated by the state. However, if you have any concerns regarding your student's

vision or hearing, please contact the district nurse who will set up a  screening for your student and let

you know whether or not further follow up is necessary.
