The citation should have a hanging indent (first line is flush left, and all subsequent lines are indented)
Your annotations should start on a new line and either be in paragraph form (first line indented) or all lines flush left with an extra line space between paragraphs.
Generally speaking, your bibliography should be in alphabetical order; however, check with your teacher as to their preferred order (e.g., by date or category).
