Sports Clearance

For each sport a student needs to have completed the Online Registration process and a completed physical from a doctor.

If you selected "No" to sports participation during orientation in August, please make sure you print and fill out the documents below.  There are two pages.  Page one is the physical signed by a doctor.  Page two  is the parent permission and acknowledgement form where you list your health insurance information.
