Data Compliance Form
Use this form to upload documents related to: Entering/New Student Data (In-State and Out of State Transfers); Exiting Student Data; Updating Rosters (Major changes to the IEP); Re-evaluation Information Submission (8 months prior to eligibility expiration); Out-of-State Transfers (Initials)

Behavior Compliance Forms
Administrators should use these forms to track discipline and out of school placement data for special education students.
Special Education Suspension Tracking (should be completed any time a special education student is suspended, this form should be completed)
ALC or Alternative Placement Tracking Form (should be completed when the student is being recommended for an alternative placement, such as the ALC due to a specific disciplinary incident)
45-Day Review Form (should be completed on or prior to the 45th school day of a special education student being placed in an alternative setting)
LRE/Homebound Compliance Form
Use this form to upload documents related to: Proposed LRE Changes (changes in placement, etc.), Behavioral Consultation Request, Homebound Request Forms, MDR Information, etc.
Homebound Application (obtain from Case Manager)
Contact your Behavior Specialists or Behavior Support Teacher for additional information/resources related to change of placement related to behavior.

MTSS- MET Meeting Request
Use the MET Request form to request a MET Meeting (14 days from request - typically following DRT Review).
Upload Files for DRT Review- Using the DRT Upload Form (MTSS Students)
*After either of these forms are submitted, schools should have a date scheduled within 48 hrs (if submitted on a weekday)

Special Services Teacher Handbook- MPSD Teacher Handbook containing all procedures, expectations, etc. for SPED teachers.
Required Meeting Attendees- Table outlining which IEP members are required for different types of meetings.
SPED Teacher Schedule 2021-2022 - Template for documenting daily schedule. Must include specific student’s served and type (inclusion, resource, etc.) Upload via Google Data Tracking Form.
PLC Sign In Sheet - Sign in sheet to be used for documenting school-level PLCs (attach agenda/meeting minutes) and upload via Google Data Tracking Form.
Key Reminders for Admin and Staff - This document was shared with admin and special education staff at the beginning of the 21/22 school year. It has important key reminders related to Special Education.
Special Education Teacher Orientation Site - This site has videos and training to support special education teachers and staff with our processes and procedures.
High Leverage Practices (pgs. 15-26)- The High Leverage Practices are a set of standards designed specifically for special education teachers to promote student learning and success.

Progress Monitoring Table Template - Google Sheet for creating a graph of progress monitoring data; automatically creates a graph based on data you input into a table.
Procedural safeguards.doc - Copy of Procedural Safeguards (English)
IEP Snapshot - One page summary of IEP for gen ed teachers (all grade levels)
Documentation of Accommodations - Form used to document that teachers/adults have received IEP information for students they work with
Record of Access Log - Used in IEP Folder to document when IEP information is accessed
SCD Guidance Document - Document from MDE to help determine if a student meets guidelines for a Significant Cognitive Disability and the self-contained/community based classroom. *Download a copy in order to use PDF form
LRE/Placement/Behavior
Sample- Bx Tracking Sheet -Tracking sheet for monitoring behavior goals (use until an online behavior program is developed and training is provided)
MPSD LRE Form - Used when considering a change in placement (inclusion to self-contained, etc.) *See Change of Placement procedures in the Teacher Handbook
MDR template - Used when a student reaches 10+ days of suspension, or considering a change of placement for discipline (consult with Behavior Specialist or Case Manager).
Re-Entry Support Plan - Used for creating partial day plans (to be completed by the Behavior Specialist or Case Manager)
FBA Consent Form - Used to obtain parent consent for a Functional Behavior Assessment (collaborate with Behavior Specialist for this form)
Homebound Procedures (21.22) - Procedures for changing a student’s placement to homebound (collaborate with Case Manager)
Teacher responsibilities for Homebound students - Responsibilities of all teachers when a student is receiving homebound instruction
BE-Well (SEL) Guidance Document - Manual for school-wide Positive Behavior Interventions and Support in schools, and MPSD BE-Well initiative
Evaluation/Eligibility
Comparable Services Form - Form used with out of state (OOS) students to document services based on OOS eligibility. Consult with Case Manager for use of this form
Authority To Obtain Information - Release form to allow for school and medical providers to transfer information/records (collaborate with Case Manager, Psychometrist, and/or nurse).
MET DOCUMENTATION FORM 21.22 - MET Form used in Eligibility/Re-eval meetings (to be completed with Psychometrist or Case Manager)

MPSD Request for OT Evaluation - Used when student has concerns with fine motor skills
