Need Home or Community Supports?
A MnCHOICES assessment determines whether an individual with a disability needs services and supports to live and work in their communities. The assessment helps decide the types of support that someone might be able to access, as well as how much support they need, and next steps to take to begin accessing services. This assessment can help determine if the individual qualifies for PCA services, or waivers like the CADI or DD. This can be done by contacting Hennepin County Front Door 612-348-4111 or contact your school social worker to complete the referral form.
Guardianship/Supported Decision Making
A person is considered a legal adult and their own guardian when they turn 18. Adults with disabilities have full rights and responsibilities unless guardianship is established. An alternative option is supported decision making. See the links below for more info.

MN state law states that an attorney is NOT required to file a petition for guardianship.
