The purpose of the County Communities on Transition is to support the successful transition of students with disabilities in Iowa, Grant, and Lafayette Counties from high school to adult life. This includes promoting opportunities for post-secondary education, employment, vocational training, independent living, community participation, continuing education, and access to adult services and agencies.
The Transition Resource Directory serves as a valuable tool for school districts throughout Iowa, Grant, and Lafayette Counties in meeting the transition requirements outlined by the Individuals with Disabilities Education Act (IDEA). It is also designed to assist students and families in identifying and accessing available county resources and support services.
The directory includes descriptions of agency programs and services available throughout Iowa and Lafayette Counties, along with contact information and eligibility criteria for each program.
County Communities on Transition meets monthly during the school year at Mineral Point High School to collaborate, share resources, and strengthen transition services for students in our schools.
