NON-PRESCRIPTION (also known as OVER-THE-COUNTER) medication may only be administered to students under 18 years old, with signed permission from a parent/guardian, including topical medications. The School-Supplied Non-Prescription Medication form can be completed electronically as part of online school registration. However, a paper copy may also be completed, signed by a parent/guardian, and returned to school.
PRESCRIPTION medication may only be administered to students under 18 years old with signed permission from a parent/guardian AND a medical provider. This includes emergency medication and so any student who self-carries emergency medication should provide an order to the school signed by a parent/guardian AND a medical provider. IF YOUR CHILD IS PRESCRIBED MEDICATION TO BE TAKEN AT SCHOOL, PLEASE HAVE THE MEDICAL PROVIDER SIGN THE Prescription Medication Permission and Instruction Form BELOW. ALL MEDICATION SHOULD BE KEPT IN THE OFFICE with the signed permission forms, unless the student has an Individual Health Plan or Emergency Action Plan that provides permission for the student to self-carry.
