NON-PRESCRIPTION (OVER-THE-COUNTER) medication, including topical products, may be given to students under 18 with signed parent/guardian permission. The School-Supplied Non-Prescription Medication form can be completed during online registration or submitted as a signed paper copy. 

PRESCRIPTION medication, including emergency medications, requires signed permission from both a parent/guardian and a medical provider. Students who self-carry emergency medication must also have a provider order on file. Please have the medical provider complete and sign the Prescription Medication Permission and Instruction Form, which may be brought to school or faxed to 608-987-3766 (Attn: Nurse Lindsey). 

All medication is stored in the school office unless the student has an approved Individual Health Plan or Emergency Action Plan allowing self-carry.
