Items Required for School

1. Immunization Certificate (up to date)

** A copy of this must be on file at school before the child's first day!!

2. Copy of official Birth Certificate

3. Kentucky eye exam

4. Physical exam (You have 30 school days to complete this from the 1st day your child begins school.)

* Bring these items in when you enroll your child. You can also fax them to TES at 270-487-9203.
