Responses to the following prompts (350 word maximum for each):
Explain what problem/issue you addressed and why you chose this. Explain the possible actions to address the problem/issue that you considered. Explain why you choose the the action you took.
Reflect back on your civic engagement project. What did you learn about yourself? How have your efforts impacted the community? How will this experience impact your future civic action?
A list of clubs, activities, and/or awards that demonstrate your civic-mindedness.
An adult reference that is not related to you (Required). Share the reference form link with the person you choose.

Here are a few examples of what you might want to include:
