ALL new prescriptions for Inj. Levetiracetam 500mg will be supplied with GENERIC BRAND (HP-Levetiracetam / Levisat)
Started by NEUROLOGY ONLY and ONLINE JKUT is required.
Verbal consent from listed NEUROLOGY CONSULTANTS is a MUST in case of incomplete JKUT form.
Each Rx and JKUT is valid for 7 days ONLY.
After 7 days, NEURO must provide new Rx & JKUT

1 Set (Box) = 2 vials of active drugs + 2 vials of diluents
Stocks are available in RESUS (2 sets) and JKUT ROOM FBW (2 sets).
RESUS may indent replacement stock at any time directly from FBW (during office hours) or A&E Pharmacy (after office hour).
Can only be started by NEUROLOGY SPECIALISTS and ONLINE JKUT form is required.
Stocks are available in ED Pharmacy (2 boxes) for acute stroke cases.
1 Set (Box) = 1 vial contains 10,000 units (50 mg) Tenecteplase + 1 prefilled syringe contains 10 ml water for injections
ED pharmacy may indent replacement stock directly from IKTAR pharmacy (during office hours).
Dose use for acute stroke is IV Tenecteplase 0.25mg/kg once (maximum dose : 25mg)
