All specimens submitted for analysis shall be accompanied by a formal drug request form UPD 1 filled in at least 3 copies.
Any amendment on the request form shall be crossed and signed.
Personnel authorized to request the drug analysis are:
Registered medical officer.
Police officer not below the rank of Sergeant or any other officer in charge of a police station.
Rehabilitation officer from Agensi Anti-Dadah Kebangsaan
Custom officer
Others (as specified by collection centre)
Completed forms shall be signed by authorized requesting officer and stamped.
Specimens shall be collected by an authorized and trained personnel from the following agencies:
a. Ward/Clinic of the hospital
b. Police department, Rehabilitation Centre and Agensi Anti-Dadah Kebangsaan
c. Other centers/agencies that are authorized by government for collection for the purpose of drug of abuse testing
Laboratory personnel shall not be involved in specimen collection.
At least 30ml urine specimen shall be collected in one bottle.
Both the personnel and the donor shall keep the urine specimen in view at all times prior to it being sealed or labeled.
At the collection site, if the volume is less than 30ml, the donor may be given a reasonable amount of liquid to drink e.g 240ml glass of water every 30 minutes, but not to exceed a maximum amount of 720ml. The second urine specimen shall be collected and mixed with the previous specimen, by the donor or the collection personnel in front of the donor.
During collection, the collection personnel shall stand close enough to the donor to see that the urine is genuinely passed out from the donor and to see that is no attempt to falsify the specimens.
After urine is collected, the bottle shall be securely capped and labeled at least as follows in front of the donor:

Date of Collection:
Time of Collection:
All urine specimens shall be sealed in the presence of the donor. Autoseal device or security wax seal shall be used.
The donor shall be asked to read and sign a statement “Akuan Pemberi” certifying that the urine specimen identified as having been collected from him/hers is in fact that specimen he/she has provided.
Personnel at the collection site shall be responsible for transporting urine specimen to the laboratory. Donor shall not be permitted to transport specimen to the laboratory.
Specimen that unable to deliver during office hours please refrigerates the specimen 2-8oC
Proper chain of custody procedures for urine collection and accurate records of individuals involved in urine collection shall be maintained.
Laboratory personnel shall not be involved in specimen collection.
For medical check-up purpose, HDOK staff shall refer to Public Health Unit (Unit Kesihatan Awam), Hospital Duchess of Kent, Sandakan.
Each referral department/unit/ward/clinic is responsible for maintaining proper chain of custody procedures for urine collection and accurate record of individuals involved in urine collection for DOA testing.
