There are many differences between high school and college for a student with disabilities - differences in policies and procedures, accommodations, courses, and the role of parents/guardians.
At the college level, students must discuss their accommodation needs with instructors and take responsibility for making arrangements for their accommodations. Many accommodations allowed in high school may not be appropriate at the post-secondary level. The most common accommodations are extended time for tests and testing in an alternate setting.
Most importantly, students must advocate (speak up) for themselves. (no IEP teacher in college)
