The transition specialist plans, coordinates and delivers a wide range of vocational rehabilitation services for students with disabilities. The Transition Specialist assists students in developing pre-vocational/ readiness skills in order to enter the workforce, as well as completing formal and informal vocational/transition assessments.
The Transition Specialist also has the role of an Employment and Life Skills (ELS) Specialist. They will advise and guide people with disabilities to find meaningful opportunities to increase their independence, employability, and social interconnectedness, provide intensive individual and group training, workshops, presentations to teach daily living skills, self-care, and vocational skills, through problem solving and task analysis. This is all done while promoting independence, working with the students to set goals for employment and independent living and create Individual Work Based Learning Plans, as necessary.
The Transition Specialist will foster independence in the choice and participation of leisure time activities, assess each student’s daily living skills and work experience level and match each individual with an appropriate internship, training or educational program. This is based on readiness and is organized to develop and implement group and individual activities which serve to integrate the lives of the students with members of their community, continually expand the program's network of employers, educational opportunities, and leisure/recreational opportunities, by performing ongoing marketing and networking activities.
Working under the direction of a Transition Specialist, the staff person in this position will:
assist in the educational and social development of students
assist specialists with conducting lessons, managing the classroom and developing materials
assist in the implementation of Individual Education Programs for the students and monitor their progress
empower students through programs designed to enhance their academic and social achievements
provide support in community-based settings (e.g. employment, recreation, leisure)
