Forms & Fees

Here is a checklist to be completed before the athlete can participate in girls basketball 

• Code of Conduct Form

• Physical Card or Alternate Year Physical Card

• Concussion Form

• Athletic Fee $25 (payable on Infinite Campus or submit payment to Middle School front office)

Below is a link to the Middle School Athletic page. It contains the forms required before students can participate in school sports. The forms need to be completed for the first sport the student participates in for the school year. If already on file for the current school year, no action is needed.
