Make sure your athlete is ready to play. ALL athletes need the following completed by the first day of practice.
If your athlete has asthma or needs to carry an EpiPen, they will need to complete the following forms. They will also need to turn in an inhaler to the athletic trainers to keep for the year.

What is an athletic trainer?
An athletic trainer is an allied health care professional.
Services provided by athletic trainers include primary care, injury and illness prevention, wellness promotion and education, emergent care, examination and clinical diagnosis, therapeutic intervention and rehabilitation of injuries and medical conditions.
Most of the time, we can get your athlete back into the game without them needing to be seen by a physician.
My child wants to play sports at Poteet High School. What paperwork do they need?
All athletes must fill out the Rank One forms online, and have a UIL Physical completed by a physician.
These must be completed and turned in to the athletic trainers BEFORE they can participate in any athletic activities.
Athletes may also be asked to complete a baseline neurocognitive exam to help with concussion management.
My child got injured, now what?
Have your student athlete come into the athletic training room between 7am and 8:30am for an evaluation. The athletic trainer will determine what next steps are needed.
If they end up needing to see a doctor, please bring the athletic trainers a doctor's note that describes what the injury is, and what restrictions they have. If we do not receive a note, we will hold them out of practice until we get a note.
Where is the Athletic Training Room?
There are two athletic training rooms at Poteet. One is located in the fieldhouse, near the Pavillion. The other is located in the high school in the hallway, by the small gym.
My child went to the doctor and has restrictions for athletic participation, who do I give the note to?
The athletic trainers!!
Is there insurance coverage for my child when they get hurt at school?
When a student athlete is injured during a school event, the athletic trainer will fill out an injury report for you to take with you to the doctor. There are instructions on this form on how to file your claim. This is a secondary insurance, meaning it will go through your primary insurance first.
This is ONLY for injuries that occur as a result of a school athletic practice, game, or event.
If you live in Dallas County and do not have a primary insurance, you may qualify for a couple programs offered through Parkland.
My child has a concussion, what do I need to know?
All concussions will be managed by the athletic trainers. If a concussion is suspected, a parent or guardian will be notified. We work closely with a Physician in the area for concussion care. Click here for more information.
My child has the flu, what do I need to know?
The school nurse and the athletic trainers communicate with each other when it comes to illnesses. If the student feels sick during the school day, they can report to the school nurse. If they feel sick during the athletic period, they can report to the athletic trainers, and we will likely send them to the school nurse.
