Staff may be eligible for a TB Risk Assessment rather than the two-step PPD skin test.

Staff who are Kaiser members may email their primary care provider to determine if they are eligible to complete the risk assessment via email. If you are determined to be low risk, you may email the TB risk assessment directly to the Kaiser Diablo Service Area Release of Medical Information desk at DSAROMI@kp.org. Your healthcare provider will complete the form and DSAROMI@kp.org will email it back to you. This process may take approximately 1 week.
