STUDENT HEALTH SERVICES

Dear Parents:

Mount Desert Island Hospital offers a School Based Health Clinic at MDI High School 1 day a week.

WHAT SERVICES ARE PROVIDED? A Nurse Practitioner is available to see your student for acute illnesses, injuries, routine exams or injections through our School Based Health Center. A Nurse Practitioner performs many of the same duties as a physician; diagnosing problems, ordering tests and writing prescriptions if needed. The Nurse Practitioner adds to the services of school nurse Tammy Und, RN, by offering the convenience of medical office visits at school including concussion evaluations using ImPACT technology as part of her assessment. The School Nurse Practitioner may consult with your student’s Primary Care provider and a report of each visit will be sent to your doctor’s office.

SERVICE ARE:

• Health Education, Wellness Promotion and Well Physical Exams

• Routine Physical Exams

• Diagnosis & treatment of acute & chronic illnesses & injuries

• Minor lab tests

• Blood draws (Lab tests processed in the hospital will be billed separately.)

• Reproductive health services

• Prescription medication ordering

• Referrals for services not provided at the clinic

WHAT IS THE COST? The Annual cost for the program is $75.00 per student. This covers all services listed above provided only at school. You may sign up for this program at any time throughout the year at a pro-rated cost. The Annual Program is open to students only. Payment is expected with the sign-up sheet before your student can be seen at the clinic.

WHAT IF I HAVE HEALTH INSURANCE? You may choose to waive the annual cost and have your insurance billed instead. We will ask for your co-pay amount before your claim is submitted and then any balance after your insurance pays will be billed to you. We do accept all insurances including MaineCare. If you have a high deductible insurance plan, you may choose the $75 Annual Program rather than have your insurance billed.
