1 Varicella (2 if first one is after the age of 13) or proof of disease from Physician
It is not required but recommended that students also get the Hepatitis B series. Most colleges have this as an entrance requirement.
The State of Maine requires that all students be immunized before entering school. Since 2005/2006 the state has required all students to show proof of having had the disease or proof of having had the Varicella vaccine.
Students will not be allowed in school without proof of immunity unless you have signed a waiver or have a note from the doctor stating why your child is not immunized.
Proof of immunity includes:
a health record that shows the student had varicella
a copy of an antibody titer
documentation of immunization from the doctor
parent/guardian interpretation with a full date of disease
