The High School clinic is staffed by Tammy Underwood, RN. The clinic is open Monday through Friday 7:30 am to 11:40 am and 12:10 to 3 pm. The phone number is 288-5011 ext 3305. 

Refer student to meet with school nurse for health concerns.

The Student Health Services Referral form is for faculty and staff communication to the school nurse. When the nurse receives this referral she will contact the referral source and follow-up with the student. The school nurse will follow up with referral source within one week unless indicated otherwise.
