Students applying for a work permit will present the following documents at the MHS main office:
The completed Work Permit Application (link)
A letter of intent from the intended employer
A copy of the student's Social Security Card
A copy of the student's birth certificate
Once the information is received, the school will process it on the State website and will email the completed work permit to the student and mail a hard copy to the employer.
