This form is for approved activities and records the number of hours you have earned and is signed by the supervisor of the service activity. Turn in the form to the SSL box in the Counseling Service Office.
This form is for activities that are not preapproved. Use this form before you volunteer to make sure the hours you earn will count toward your SSL requirements.
This form is for recording ongoing service learning experiences. Use it to record your hours each time you volunteer.
