In order to complete the Transcript Request Form, you must know the following information for each school you are applying to:
Name of the school (City and State)
Application type (mail, common app, e-docs)
Decision type (early decision, early action, regular decision, priority)
Deadline
Once you have completed this information, you and your parents must sign (e-sign/type) the waiver at the bottom and then click the “Submit” button in the top right corner. Please note, this should be submitted one month prior to your first deadline.
If you are applying to more than nine schools, please submit an additional transcript form.
