Receipt and Bond for Band Instruments This form is for students who will be using MCPS owned instruments.Please fill out and print forms can be provided in class for students who are unable to print. There is no rental fee for use of school owned instruments, however students are responsible for purchasing items from assigned supply list. Ms. Benjamin will fill out the Make, Serial Number, and School Instrument No. sections of this document.
