All DMV facilities require you to sign up for appointments online, you no longer can just show up randomly. When you are signing up to go get your driver's license, you must make appointments under "IN CAR DRIVING TEST." Even if you earned a driving waiver you must sign up this way. The DMV's randomly do what they call "SPOT CHECKS," which means even if you earned a driving waiver you MAY still have to do the driving test at the DMV to get your license. We as a school/program have NO CONTROL over when the DMV decides to do their "SPOT CHECKS."

In order for students to receive their Drivers License before 18 years of age, they must..
Be at least 16 years of age.
Have a valid driving permit for at least 9 months and 1 day.
Have completed and passed both Drivers Education classroom (30 hours of classroom instruction and 6 hours behind-the-wheel with an instructor)
You must have your completion letter from Minooka HS stating that you successfully completed the Driver Education Program (all students received this after completing the course).
If you earned a driving waiver you must have your waiver with you (all students that earned the driving waiver received the waiver after completing the course).
You must have your 50 hour log sheet completed and filled out.

Affidavit/ Consent for minor to drive form is attached below. A new law requires parent or legal guardian to provide written consent at the time of the application for a driver's license, verifying that the applicant is sufficiently prepared and able to operate a motor vehicle safely. A responsible adult may take the place of the parent or legal guardian only after the Affidavit Consent for Minor to Drive has been completed and notarized.
