Immunization Information

You are encouraged to have your child immunized well in advance of school starting in August to avoid the late summer rush at immunization clinics.

For immunizations contact your doctor, clinic, or nearest public health department.

WAIVERS FOR EXEMPTION

If you are choosing to waive any or all immunizations for religious, health, or personal conviction reasons, an immunization waiver must be completed and in the student’s school file. 

Please contact the nurse if you need assistance in obtaining a waiver form.

All vaccinations that were administered in Wisconsin are available to the Health

Services Team, and documentation is not required. 

Documentation is required for

any vaccines administered outside of Wisconsin.
