Every day vaccination saves lives and makes it possible for Victorians to live free from the illness and disability caused by many infectious diseases.
The Secondary School Vaccine Program offers free vaccines each year to help make sure students and communities are protected from diseases.
Students will have the opportunity to receive the following:
Year 7 Students
Diptheria-Tetanus-Whooping Cough (Pertussis)
Human Papillomavirus (HPV)
Year 10 Students
Meningococcal ACWY
Local councils coordinates with school and deliver the program. Schools distribute the information and consent card booklets.
The Public Health and Wellbeing Regulations 2019 authorises secondary schools to provide their local council with student and parent/guardian contact information for any student scheduled to receive a vaccination. This information assists local councils to follow-up parents/guardians of students with incomplete or missing consent cards.
Schools provide the following basic student and parent/guardian contact information:
Student name, gender, date of birth, year level, class, language(s) spoken at home
Parent/guardian names, phone numbers, email and postal addresses
To ensure all Year 7 and 10 students are offered the opportunity to access free vaccinations at school.
To improve communication with parents/guardians regarding their child’s vaccination needs.
To reduce the administrative burden on school staff to follow-up consent cards.
Your information will be provided to Wyndham Council in March 2024. If you do not wish for your details to be provided, please advise the School Nurse Kathleene Quiambao by calling 9741 4202 or in writing (Kathleene.Quiambao@education.vic.gov.au).
Read the information provided in your child’s consent card booklet and complete all sections, regardless of whether or not you consent for your child to be vaccinated as part of the school-based program.
Detach Part A of the consent card booklet with all fields completed, and return to school.
Students who miss immunisations are still eligible to receive free immunisations at their local council immunisation service. Missed immunisations can also be arranged at a GP and in some pharmacies, however there may be a fee associated with administering the vaccine.

First aid refers to medical treatment required for the immediate relief of an acute symptom usually administered immediately after the injury occurs. It often consists of a one-time, short-term treatment and requires qualified First Aid staff to administer to ensure staff and student safety.
If a student has any chronic medical condition (e.g., Asthma, Anaphylaxis, Diabetes, Epilepsy etc) details of the condition should be noted on their enrolment form/school record. Any relevant medical and supporting information must be provided to First Aid.
If you feel unwell during the school day, you can ask your teacher for a pass to visit Wellbeing Hub/Secondary Sick Bay Office.
School Nurse or First Aid Officer will assess you and contact your parent/carer when necessary.
Students may not leave school premises unless permission has been provided by the parent/guardian.
Schools must obtain written advice on a Medication Authority Form for all medication to be administered by the school. It is recommended that the form be completed by the student’s medical or health practitioner ensuring that the medication is warranted. However if this advice cannot be provided the principal may agree that the form can be completed by parents or carers or adult or independent students.
We encourage parents or carers to consider whether they can administer medication outside the school day, such as before and after school and before bed.
Form can be obtained from the School Nurse or First Aid Officer (located in Wellbeing Hub/Secondary Sick Bay Office) or you can call the front office and request for the form to be emailed to you.
Once completed, return the form and medication supply to First Aid Department.
All medication (including EpiPens) needs to be in date and in the original container with the original label including the name of the student, information on the dosage and time to be administered
First Aid Officers will inform parents/guardians when stored medication is due to expire to request a replacement.
In relation to medical conditions (other than Anaphylaxis) a Student Health Support Plan will need to be completed by parents/guardians and the school.

If support to complete the form is required please make an appointment by calling Administration to come and speak to someone.
A Student Health Support Plan must be completed for a student with an identified health care need, other than anaphylaxis or an allergy (see Anaphylaxis and Allergies about the requirements for those conditions).
To develop a Student Health Support Plan, schools must:
work with families to develop the plan, guided by advice from the student’s treating medical/health practitioner
include actions in the plan as to how the school will support the student’s health care needs
communicate regularly with parents about the student’s health care needs at the school and update the plan if necessary.
Students with Asthma, Diabetes, or Epilepsy need to provide their (condition specific) health management plan to the school, and can have a Student Health Support Plan developed, which outlines how the school will support the student’s health care needs.

A copy of the students Diabetes Management Plan must be obtained from the parent or carer and held by the school. This plan outlines the student's management plan and the emergency procedures to be taken in the event of a low and high sugar levels.
Parent/carer's responsibility to provide the school with a copy of their child's updated Diabetes Management Plan with up-to-date photo of the student and to inform the school if their child's medical condition changes. Diabetes Management Plan has to be reviewed annually by your medical practitioner.
Parent/carer(s) must supply hypo kit, medication and all other supplies need to be provided to First Aid.
If the student is self-managed, it’s always a good idea to have their supply kept in First Aid in case of emergency.
A copy of the students Epilepsy Management Plan must be obtained from the parent or carer and held by the school. This plan outlines the student's known triggers and the emergency procedures to be taken in the event of a seizure.
Parent/carer's responsibility to provide the school with a copy of their child's updated Epilepsy Management Plan with up-to-date photo of the student and to inform the school if their child's medical condition changes. Epilepsy Management Plan has to be reviewed annually by your medical practitioner.
If student has prescribed emergency medication to manage their seizure, an Emergency Medication Management Plan and medication (e.g. Midazolam, Clobazam, Clonazepam, Rectal Valium) must be supplied to school. This plan has to be completed and signed by a doctor and has to be reviewed annually.
The College will call an ambulance immediately if:
we do not know the student
it is the student’s first seizure
there is no epilepsy management plan
a serious injury has occurred
the seizure occurs in water
we have reason to believe the student may be pregnant
other factors outlined on the epilepsy management plan are occurring
A copy of the students Asthma Action Plan must be obtained from the parent or carer and held by the school. This plan outlines the student's known triggers and the emergency procedures to be taken in the event of an asthma flare-up or attack.
Parent/carer's responsibility to provide the school with a copy of their child's coloured Asthma Plan with up-to-date photo of the student and to inform the school if their child's medical condition changes. Asthma Plan has to be reviewed annually by your medical practitioner.
Puffers (such as Ventolin) and spacer must be supply to Wellbeing/Secondary Sick Bay Office.
A copy of the students ASCIA Action Plan for Anaphylaxis (prepared by a medical or nurse practitioner) must be obtained from the parent or carer and held by the school. The plan outlines the student’s known severe allergies and the emergency procedures to be taken in the event of an allergic reaction.
Parent/carer's responsibility to provide the school with a copy of their child's coloured ASCIA Action Plan for Anaphylaxis with an up-to-date photo of the student and to inform the school if their child's medical condition changes. ASCIA Anaphylaxis Plan has to be reviewed annually by your medical practitioner.
EpiPen and antihistamine medication indicated on Anaphylaxis Plan must be supply to Wellbeing/Secondary Sick Bay Office.
An ASCIA for Allergic Reactions (Green Plan) is a document completed by the student’s medical practitioner and is for students who have mild to moderate allergies.
Parent/carer's responsibility to provide the school with a copy of their child's updated coloured ASCIA for Allergic Reaction (Green Plan) with an up-to-date photo of the student and to inform the school if their child's medical condition changes. ASCIA for Allergic Reaction Plan has to be reviewed annually by your medical practitioner.
Antihistamine medication indicated on Green Plan must be supply to Wellbeing/Secondary Sick Bay Office.
