In this video clip—I used the first Death Star’s destruction because it shows the lack of risk analysis done by the Empire. The reason being is because the Empire clearly left an opening vent for Luke Skywalker to search an exploit. The Empire did not do enough research, nor did they think enough that an opening vent could lead to the destruction of the Death Star. The failure to recognize risk.
The Empire needed to carefully consider how completed the project—the Death Star—is going to function in the real world—and did not consider any other situations. If our project were to have a metaphorical exhaust port—we need to know about it—even if it’s a few meters wide. Therefore, risk assessment and a thorough risk assessment was much needed to identify any risks or threats that could pose any sort of problem, unlike the Empire, we can learn a few mistakes from them.

A severe lack of proper risk management is observable in this scene from the 1999 film October Sky. In this scene, Homer Hickham and his friends attempt to launch a model rocket several miles by packing it with excess amounts of explosives. They did not, however, undergo proper risk management by assessing, evaluating, or mitigating any possible risks involved. This is clearly noted by dialogue "Should we get behind something?" as the rocket's fuse had already been lit. By then it was far too late to undergo any proper risk management or mitigation of risks, and the subsequent explosion caused considerable damage to the fence where the rocket was mounted. This could have been prevented by:
Identifying risks - What if the rocket explodes? What if the fuse timing was more or less than expected?
Determining likelihood of outcomes - How likely is it that something could go wrong? Nobody involved was experienced in such a project, and any prior knowledge was minimal. In combination with the fact that the model rocket was not designed for excess amounts of explosives, the outcome of an explosion would seem highly likely.
Determining Impact of outcomes - Would the nearby area be damaged or destroyed? Would anyone involved be injured or killed in the event of an explosion?
Determining response - What could be done to mitigate the risk of an explosion? What could be done to minimize negative impact in the result of an explosion?

I chose the matrix famous pile scene because it represents a good amount of risk between two choices.
The scenes start off where Neo has been introduced to Morpheus by Trinity and Morpheus has a conversation with Neo about the matrix and so he talks to him about two pills of the truth, the blue pill just lets Neo stay in the matrix and believe whatever Neo wants to believe while the other pill lets Morpheus show Neo about the deep truth of the matrix. Ultimately Neo ends up taking the red pill and then is taken to a room of computers.
Project risk is important because often in projects there is going to be a lot of risk and it is good to know and choose the correct risks are important and so that the project manager can choose the correct choice and know the certain risks of the choice. In this example Neo was presented a couple of choices, the blue pill that gives him no risk as it just makes go back as if nothing had happened, While the red pile lets Neo discover the truth of the matrix by Morpheus.
