C/O - Care Of
It is used in addressing communication, indicating that the message or item should be delivered to or managed by a specific person or organization on behalf of another. For example, "C/O Dr. Smith" would mean the letter is being sent to Dr. Smith for their attention.
CB - Call Back
Call Back is a term used to indicate that a patient or healthcare provider will return a phone call or respond to someone who previously contacted them.
CONT - Continuous
It refers to something that occurs without interruption, such as continuous monitoring, continuous infusion of medication, or continuous care.
DC - Discontinue
Means to stop or cease something (e.g., treatment, medication).
DC - Discharge
Refers to the release of a patient from a healthcare facility after treatment.
EXC - Exchange
EXC stands for Exchange. It is used to refer to the process of swapping or replacing something, such as exchanging medical equipment or exchanging one form of treatment for another.
FF - Focused Follow-Up Visit
A Focused Follow-Up Visit is a follow-up appointment or consultation where the healthcare provider specifically addresses particular health concerns or issues related to a patient's condition.
INS - Insurance
INS stands for Insurance, often referring to health insurance that covers medical expenses.
INV - Invasive
INV stands for Invasive, referring to procedures, techniques, or devices that involve entering the body, often through incisions or punctures, such as invasive surgery or invasive monitoring.
LVM - Left Voice Message
Left Voice Message refers to a situation where someone attempts to contact a patient or healthcare provider but is unable to reach them directly.
MCAL - Medi-Cal
MCAL refers to Medi-Cal, which is California's Medicaid program, providing health care services for low-income individuals and families.
MCR - Medicare
MCR stands for Medicare, a federal health insurance program in the United States primarily for individuals aged 65 and older, or for certain younger people with disabilities.
MD - Medical Doctor
MD stands for Medical Doctor, a professional title for someone who has earned a medical degree and is licensed to practice medicine.
NIV - Non-Invasive
NIV stands for Non-Invasive, referring to procedures or treatments that do not require entering the body or breaking the skin. For example, non-invasive ventilation (like CPAP or BiPAP) provides respiratory support without the need for an artificial airway.
NOC - Nocturnal
NOC stands for Nocturnal, which refers to activities or conditions that occur at night. For example, "noc breathing" would refer to breathing patterns observed during the night.
PORT - Portable
PORT stands for Portable, referring to equipment, devices, or services that can be easily moved or transported, such as portable oxygen or portable monitoring equipment.
PRN - As Needed
PRN is derived from the Latin phrase "pro re nata," meaning as needed. It is used in medical prescriptions to indicate that a medication or treatment should be administered only when necessary, rather than on a regular schedule.
PT - Patient
PT stands for Patient, referring to an individual who is receiving medical care or treatment.
PU - Pick Up
PU stands for Pick Up. This typically refers to the act of retrieving or collecting something, such as picking up a patient for transport or picking up a medical device or supplies.
QTR - Quarter Visit
A Quarter Visit refers to a scheduled appointment or visit that occurs once every quarter, typically every three months.
RT - Respiratory Therapist
A Respiratory Therapist is a healthcare professional who specializes in the treatment, management, and care of patients with breathing or cardiopulmonary disorders.
S/O - Switch Out
S/O can stand for Switch Out, which generally means transferring or replacing a patient, service, or responsibility from one entity to another (e.g., switching out a caregiver or equipment).
STAT - Stationary
STAT is an abbreviation for immediate or urgent (often derived from the Latin "statim"). It typically indicates that something needs to be done as quickly as possible. However, in some cases, stat can also be used to mean stationary, depending on the context, though its most common medical usage is urgent action.
TRT - Trach Tube
TRT stands for Trach Tube, which is a short form for Tracheostomy Tube. It's a medical device inserted into the trachea (windpipe) to assist with breathing in patients who cannot breathe adequately on their own.
TRX - Treatment
TRX stands for Treatment, referring to the medical care or procedures given to a patient for managing or curing a condition.
W/O - Without
W/O stands for Without. It’s used to indicate that something is missing, absent, or not included in a particular context. For example, "patient W/O pain" means the patient is without pain.
