Athletics Forms
AHSAA Participation Agreement Form
- Students are required to have all athletic forms submitted BEFORE tryouts. Forms include the AHSAA Physical Form, AHSAA Participation Agreement Form, Athletic Insurance Form, and a copy of the student's Birth Certificate. Additionally, students must complete the NFHS Sportsmanship online course and submit the certificate of completion.
