"This is the documentation that a applicant is asked to review and sign at time of intake. All pages need to be signed and the last page ask for Students signature and date. The last page is letting the student know that Vocational Rehabilitation will not discriminate them in any way. They need to sign this page stating they understand this policy.
That is the paper work for the intake process. Once we are done we request documentation from the school district (IEP’s, Psychological Evaluation and Eligibility Team Documentation) and Doctors information or outside counseling or Psychiatry or Psychologist reports.
We have 60 days to determine them eligible for VR services. If I receive the documentation quickly we can do it sooner. Then I must have them in an Individual Plan gor Employment in 90 days or have them sign an extension.
Once in plan we can move forward with all services as services providers are available."
"The financial forms. We need to complete this preferably at time of intake but must be done before we put a client into an IPE and pay for any services. Please ask your VR counselor if you have any questions. It is usually the most complicated part of our process. However, VR counselors will handle all of this. The client or parents just need to supply their W-2 Adjusted Gross income or proof if they are receiving TANF, SNAP (Food Stamps) or SSI or SSDI."
"Here is the VR process. What a person progresses through to reach a successful closure with Vocational Rehabilitation. I also attached the parental consent form that needs to be brought to intake for any student under 18 years of age."
"The attached form is the intake checklist. All of these items must be reviewed and brought to the Intake appointment before we can move forward with the intake. All of these items are important most of all, ID and Social Security Card. Proof of income needs to be provided as well before we can move forward with the Intake Interview."
"This is the application packet that must be completely filled out at the time of the Intake Interview."
Consent for Outside Agency is a requirement in LCSD for VR to be invited into the IEP meetings.

Tips, checklists, and tools to support successful work outcomes for SSI youth
As a vocational rehabilitation practitioner, you play a key role in helping youth who receive disability entitlements to make informed choices about work, earning and their benefits. You also play a key role in supporting both youth and their families in not only understanding the impact of earnings on benefits, but also how to develop pathways to greater economic self-sufficiency in support of their life goals. As you prepare youth to join the workforce of tomorrow, access to reliable and accessible information about benefits and earnings is essential—as is access to qualified benefits and work incentive planners.
That's what the Vocational Rehabilitation Practitioner’s Just-in-Time Toolkit is about. Each of the ten tools below are based on information most essential to assist you in supporting youth who receive disability benefits and their families in navigating a path toward successful employment. Each tool is based on a specific knowledge you will need to support a youth in understanding and each has been designed to be used in about five to ten minutes. If you choose, each tool also contains other links to test your knowledge, print out a one-page sheet of reminders or pursue further resources.
