Use this form to order additional textbooks, core curriculum replacements, or any other current materials we store in our warehouse. The top portion of the form should be used to order textbooks, workbooks, TE materials for Core and Supplemental/intervention programs. The bottom portion of the form should be used to order materials from the IMC Circulation library (class novel sets, science modes, STEM kits, Reading Rainbows). To know what we have in stock, you can click on the IMC/Warehouse site on Destiny and look at the current catalog.
Use this form for the pick-up of current/obsolete and other materials that are requested from sites. This includes library books for processing, excess chromebooks that you send back after the “swap” of devices at 7th grade and 2nd grade, weeded library books, transfers to other sites, etc.
Elementary Textbooks: Textbooks are housed in the teachers' classrooms for elementary students. Only some textbooks are barcoded( Tip: Click on the PDF titled "Textbooks Check in-Check out Guide". It is important that you view that list to know which textbooks need barcodes. If textbooks are not barcoded, you will have to barcode them using the "5,000" digit barcodes. If you do not have any elementary textbook barcodes on hand, email Lupe at gulopez@lodiusd.net, and she will send you some. You must check out textbooks at the beginning of the school year and check in textbooks at the end of the school year. This helps the IMS department keep track of inventory. If students take textbooks home, you should check them out to the student under the student's account. If the textbooks are left in the classroom, you can check them out under the teacher's account.

Teacher Materials can be checked out as "Unlimited"
Teachers who leave the site should get all of their teacher materials checked in at the end of the school year. Leave all Teacher Materials in the room ready for the next teacher. Make sure to check out the TE to the new teacher at the beginning of the new school year.

If textbooks are not barcoded, you will have to barcode them using the "1,000" digit barcodes. If you do not have any Middle school textbook barcodes on hand, email Lupe at gulopez@lodiusd.net, and she will send you some. You must check out textbooks at the beginning of the school year and check in textbooks at the end of the school year. This helps the IMS department keep track of inventory. Core novels will be checked out every quarter for the ELA classes. Please keep track of the condition of the core novels, and add fines to students' accounts if they are too damaged to be circulated. You can attempt to repair them, or you can weed out core novels at your discretion and order replacements using the *Current Materials Order Form* linked at the top of this page. Always make sure you have enough inventory for all the ELA classes before it is time for that novel to be checked out.
