3. Click Next

4. Click on LJISD Pre-District Entry Form 3/7/2026 Link 

5. Make a COPY of Entry Form 

(Click on the File tab on upper left hand corner, then make copy)

6. Fill out Entry Form  

7. Upload the Entry Form
